Cognizant Technology Solutions — didn't received the offer letter

Hi,

Please help me out in below.

I have attend the interview on 12-apr-2019, i have completed all the rounds and hr discussion done on same day. Hr asked me to sign the form since i was instructed like i will be receiving the offer letter within 10 days. Hence i am not received any offer letter so far.
