Cognizant Technology Solutions — Unable to change the DOB in PAN declaration form at the time of pre joining process

I am facing an issue with my pre joining formalities with Cognizant.
In the PAN declaration form my DOB is incorrect. The form is non editable and i am not able to make changes into the form.

The DOB reflecting in the form is 09-Apr-1995 whereas my correct DOB is 04-Sept-1995.

I tried connecting to my HR but there is no response from the HR and onboarding India process team.

Kindly help me how can I proceed further for smooth onboarding in Cognizant
