Cognizant Technology Solutions — Unable to enter my input in Legal last name in CDF 1 form

I am not able to enter my input in Legal last name in CDF 1 form which is sent by cognizant talent acquisition. I have to submit this as soon as possible. Kindly help in this. I am not able to submit the forms.
