Cognizant — Unable to apply for new food card

Address:"Buniyad Society", 54/6, Sectore 21, Scheme 10, Yanumanagar, Next to Green Fresh Mart, Nigdi, Pune

I have lost my HDFC food card. I am unable to reach customer care through call. Could you help how to block the card and get the new food card.
