Dear Sir/Madam, This is in reference to my policy plan is JIBAN SARAL, It was registered in the month of Nov 2009 from your 31C branch in Connaught Place, New Delhi. I have paid 3000.00 as its 1st premium for one yearly. (yearly plan). As I have misplaced its receipt, therefore I request you to please send me or mail me the duplicate recipt with all other policy document. Till date I didn't recive any policy papers also. Please consider my application & do the veedful at earliest. I have taken one BIMA GOLD Policy on[protected] for Rs.200000/- vide Policy No.[protected] in the name of Kailash Chandra Das from your Bhubaneswar Branch-1 No.583 under Cuttack Divisional Office. As it is a money back policy every 4 year after I will get Rs.20000/-. The first money back return I would have received on or before[protected]. But since now I have not received any cheque or credit through ECS or any information from your office.

Are you thinking that this is a good service rendered by your office to the Policy Holder.

Kindly confirm the above at the earliest.
