Dear Sir/Madam,


Sub: closure of ECS of LIC of India policy No.[protected] premium of Rs. 10208/- Per Month with Branch LIC OF INDIA, Mumbai Branch No. 922.

This is to inform you that, my LIC of India Policy No.[protected] of plan 165/28/28 Jeevan saral with profit sum assured Rs. 25,00,000/- (twenty five lacs) Date of commencement 28/09/2008.

I have already submitted a letter for partial surrender of policy S A Rs. 25 lakhs to 5 lakhs and to stop ECS payment of premium on dated 28/05/2012 to LIC of India Br. 922 and surrender value Form with revenue stamp with my signature on date 14/06/2012.
But LIC of India Br. 922 branch officer has not been able to do this and has informed me to contact PCMC jeeven seva annex to stop payment of monthly ECS premium of Rs.10208/- for policy no.[protected].

I request to you cancel the ECS for above policy.
