It is to inform you that i am a policy holder of policy no[protected]opening Dt;26/02/2004.At that time i was working as Assistant District Welfare officer in Sub-collector office,koraput(orissa).I have been retired from Govt.service w.e.f 30/11/2007 and residing in my native place in the following adress.I have already submited my enclosement details to IPPCELL,JEEVANDEEPBUILDING,EXIBITIONROAD,PATNA.But i have not been recieved the mail or annuity till date.
