[Resolved]  Ration Card — add name

i want to add my child's name in the ration card but he is knw 15 yrs. old and was born in 1995
his name in his birth certificate is Ankit balji patel where as i want to put it in a ration card where the name is kara sava vaidh
my name from balji patel is already changed to kara vaidh in a gazette hence wat all documents are required for that i wanted to knw kindly help me
i am new to this place.....its a request
