I need my address to be changed in my ration card,We are reaching the ration shop evry now and then to receive the form for applying change of address but no proper response,We are worried whether our ration card would be listed in the bogus one..

I am a citizen of India of A.P, I Kum. K. Vijaya Lakshmi, (Spinster/ Un-married)D/o. Late K. Laxmaiah, (Ration Card No.[protected], Circle No.IV, Ward No.5) shifted the house from H.No.5-9-1018, Gunfoundry, Hyderabad - 500 001, to H.No.10-1-397 to 412, Flat No.205, sri Laxmi Devi Apts, Veernagar, Chinthal Basti, Near Vijaya Marry Hospital, Kharitabad, Circle No.VII, Ward No.10 Hyderabad - 500 004.

I request you to issue me Ration Card with New Adress i.e., H.No.10-1-397 to 412, Flat No.205, sri Laxmi Devi Apts, Veernagar, Chinthal Basti, Near Vijaya Marry Hospital, Kharitabad, Hyderabad - 500 004.
