Dear Sir,
With reference of above subject & reference numbers, I have to inform you that myself worked with M/s Tejpal Motors Pvt. Ltd. N.H, No. 8, Balitha, Vapi- 396195 vide Establishment code no. GJ/VPI/46796 for the period from 12.08.2005 to 05.12.2007. During my job period the employer was deducted E.P.F money from my salary. After my resignation I have contact with employer for settlement of my P.F money & request for attestation on Form no. 19 & 10C. But they are not attested my forms.
So I had logged Grievance vide no. EPFO/E/2009/1407 & 1438 dated 30.01.2009 in P.F department. On behalf of my grievance P.F office, Vapi had sent to me the Form no.19 & 10C vide letter no. GJ/SRO/VAP/A/C/II/337 dated 06/02/2009. After receiving of above forms I had sent filled form to P.F office Vapi dated.16.02.2009 which is officially received by dated 17.04.2009. But I had not received any response from their side.
Then I had logged 2nd Grievance vide no. EPFO/E/2009/11210 Dated 25.08.2009, in this grievance I have got reply from P.F office, Vapi vide letter no. Letter no.GJ/EPFO/SRO/VAP/A/C/II/831 dated 26.08.2009 with remarks “YOU ARE AN EXCLUDED EMPLOYEES” & your salary is above Rs.6500/- & not allotted P.F. account number, and also returned my filled form no.19 & 10C which is attested by the employer.
Sir, I have to inform you that my starting salary was Rs.6000/- per month & employer was deducted E.P.F amount from my salary.Sir, I request you kindly look into this matter & do the needful at the earliest.
