What is the procedure for getting a duplicate one

right add. is - (H.NO-30A, GALI NO-7/3 SHAKTI VIHAR MEETHA PUR BADARPUR NEW DELHI-110044)

voter-ID — Incorrect Date of Birth
I have been waiting for my voters id for 5 years and now i recieve is with incorrect details. My date of birth is XX / XX / 1987 instead of 06 / 05 /1985. And also my Fathers Name is M R Leslie Gomez instead of Mr. Leslie Gomez...Pls make the necessary amendments and send me a new id card and i hope it does not take another 5yrs now...Thanking you
