Voter ID Card — Wrong Address and Date Of Birth printed In The Voter ID Card

My name is SOUVIK MAJUMDAR, i a m from Kolkata,My complain is that my Addess as well as my date of birth has been misprinted in the voter ID Card.My ID no Is GGC4333373. My actual Date of Birth Is 31 st Dec 1978 and my address is CB/11A,Railpukur road,P.O:Deshbandhunagar,P.S: Baguiati,Baguati,Kolkata-700059.But To my Utter astonishment what i find is that my date of birth has been mentioned as XX / XX / 1979 ,and my address has been mentioned as : Raghunathpur Dakshin Parar(PurbaAnsha) 21 Rajarhat North 24 paarganas 700059. Now may i request you to lodge a complaint regd this and send me complain number if any.And tell me the procedure to get it corrected.
