My self girish kulkarni having bank account in basavangudi branch with ac no: [protected]
I have swiped my rupay card (Atm card) at prabhakar kore hospital, kle, belagavi pos. On 21.05.2018 inr 30, 531 at 3:49pm. (Rupees thirty two thousand five hundred thirty one)
The amount is deducted from my account but not transferred to prabhakar kore hospital, kle, belagavi.
I request you please rectify and credit back to my account.
