I am unable to fill the claim form for following Karvy web portal, because when i am trying to provide my Golden Forest A/c number, it is showing me 'Invalid Account Number'

Date of issue: 19/09/1997
Amount Invested: Rs.1000/-
Maturity Date:13/01/2022
So, requesting you to help me to received my amount as soon as possible.
