Golden Forest India — not able to get my policy return

I have golden forest policy with name mithlesh rathore from jabalpur. Till now i haven't received my return. Because of long time i have lost my policy papers and i am not not able to register on refund process initiated by supreme court. Kindly help me to register on refund process. Kindly provide me account no. Of policy based on my name in policy and location so that i can get my refund.
