Haveus Solutions Private Limited — pf transfer issue

Hi team

I am struggling to transfer my pf since many years. When i tried to email on the company address nobody answers me on call or mail.
My name is different in the pf details raised from haveus, it is showing neha ranoliya while i use only "neha" name in pan, voter id etc. Because of that my pf transfer form get rejected.
Now i need the employer signature in "joint declaration" form to transfer my pf in current account. Please assist and share the current address of haveus company or correct my name in the raised pf.
