Kindly amend in your records and rectify the mistakes and reissue the Hero GoodLife Program Online Acknowledgement cum Declaration Receipt and Tax Invoice with my correct particulars as mentioned above. This is urgent as the correct particulars are required in case of an accident insurance claim.
