Income Tax Department — Pan card lost

I have lost my purse in train between grant road to dadar on dt.[protected] in which i have lost my original pan card no. Atnpm5944p, duplicate copy of aadhar card & my photo. I want to request you please don't allow anyone to misuse of this pan card & other documents. I also want to apply duplicate pan card so please inform me which documents i want to submit.

I paid advance tax through internet e-payment from dated 14.02.17 pan no AGVPV9198D reference no: IK00CGQUH4 debited a/c [protected] rs 15625/- but did not come tax payer counterfoil please kind your information.

Dear sir i have submitted an application of new pan acknowledgement no. is n-[protected] on 11 jan 2017 and i did not get any progress till now .please take some action on it.
