I am vijay shah. My policy number is p/900000/01/2017/000007 and my id number is [protected] my claim number is cli/2018/900000/0251167.

My mother admit in long life hospital, ahmedabad on dated[protected] we have applied for cashless but we have non received of any amount for claim from your company. I seriously request you to look into the matter and release the claim money asap.

As a lower class people we cant afford to wait so much for the claim money.
