Lic Of India — No annuity no

i am manish jaisinghani and i have worked in a company for 17 years, the company send me form for filling and there is no annuity no./master policy no., only they gave me a/c no. 320069 and LIC id no. 1274. How can I get annuity no. or master policy no. when try to contact lic office they are not responding . plz help
