Dear Sir,

I have paid Rs 13944/- as premium of LIC Policy no.[protected] on 08/02/2016 trough ATM Card of OBC A/c no. [protected]. But I have not get any receipt by mail or otherwise till date. This amount has been debited from above a/c no. on 08/02/2016.

Please take necessary action in this matter and provide me receipt of above amount.

Dear Sir,

I have submit following complaint about no receipt genereted against online payment to LIC on 16/02/2016. but till date I have not received any receipt against that payment done on 08/02/2016. Please consider it as urgent and resolve it as soon as possible.

(I have paid Rs 13944/- as premium of LIC Policy no.[protected] on 08/02/2016 trough ATM Card of OBC A/c no. [protected]. But I have not get any receipt by mail or otherwise till date. This amount has been debited from above a/c no. on 08/02/2016.)
