Ltimindtree — Mediassist - Claim repudiated despite of sharing all necessary documents

I have submitted a claim 33023264 including all documents physically and virtually both, however 80% of my claim amount reject saying "No Documents Available". There were several follow-ups done with Medi assis customer care but they are not at all trained to understand and solve basic problems.
