Mdindia Healthcare Services — domiciliary treatment claim (policy number 500100/28/16/p[protected] non receipt of payment

I have sumbitted the above said domiciliary claim on dated 29.05.2017 for rs.8799/- under mdindia id no. Md15-[protected] and employee id no. J-10486. I have already submitted you the original reports/receipts/bills etc.
But till date i have not received the payment of the claim.
You are requested to clear the claim and send the proceeds in my ca a/c no.[protected] in punjab & sind bank batala (B-0094)
