While contacting to MD India Toll free number on 10th Dec 2016 at 5.30 pm they says that I have to submit the printed receipt. I have explained to the lady executive of MD India that Physiotherapist cannot provide me the printed bill receipt. So Md India lady executive has advised me that I have to obtain a undertaking letter from physiotherapist in which she will give an undertaking that She provides the Receipt on the plain format alongwith her Registeration Stamp to all their patients and she doen't have any format other than this.
