Mediassistindia Tpa — help for providing hospital discharge summary to enroll aadhar card-mediassist

Dear Sir/Mam,

This is to inform you that “GOD” blessed me a BABY GIRL on 3rd of Nov 2018 with normal delivery. My wife was admitted on 3rd of Nov at Sanjeevani hospitalat panipat and she discharged on same day in evening.
Refer Medi Assist ID:- [protected], my policy no. 070300/48/18/BPS/KSA/WT-NonSez_Base_Plus_EX and my claim no. 97208687.
i have submitted my all original documents i.e Hospital discharge summary, dr. investigation report and all medicine bills with lab test reports.


I required Original Hospital Discharge summary to enroll aadhar card of my daughter.

I will return once aadhar enrollment done. I already having photo copy but aadhar center person not accepting photo copy . so requesting to you kindly help..
I assure I will return back and numerous mail sent to mediassist but they are denying to return back orignal documents but i assuerd that will return the same once aadhar registration done.
problem is without discharge summary i am unable to do birthcertificate process.
