Municipal Corporation Of Delhi — Issuing Death Certificate Application Number 0221-[protected]

Dear Sir/Maam,
I have applied online application for Death Certificate from MCD online site. Three to four times application has been rejected without any contact.
My mother aadhar card address was different to current address. She was expired in Burari address. In Shaamshan Slips address is mentioned as per Aadhar Card.
Please issue my Mother Death Certificate. I thanks to you.
Below are application number which is already rejected. Even I have paid fees.
