Nokia India — date of leaving

Myself saichand. I am from nizamabad. I have worked for nokia mobiles india under payroll of ranstad india pvt ltd. I have resigned for nokia in the month of june and applied for my pf in last week, it's not showing date of leaving (Date of exit) in that. Due to this iam unable to apply get my pf. So please check the details given below and enter the date of exit as much as possible.
