ONGC — Ongc scholarship

I am 2nd year medical student .My name was metioned on the 2019 scholarship list. And i also got my scholarship but i want to ask you if i have to fill ongc scholarship form again ? or i dot hve to fill form again and again and i will receive scholarship annually for 5 years of my mbbs course?

I have selected ongc scholarship Sc/st 2019 but i have not received the scholarship amount till now. And also i have sent the marksheets for 2nd installment and still i have not got any response from ongc.
