Pacl India — claim form

My doc pacl expire date agreement completed 30/9/2014
So i submit all documents in pacl office villupuram (Tamilnadu).
Consideration amount:50000.
But i am paid money for 5 or 6 installment (Per quarter rs 2335.00).
My question is how to know exact amount i am paid against premium my policy.
Document enclosed. Give me a solution

When I was trying to register refund form unable to register. It says "This Pacl number does not exist as per our records" but I have got the documents and receipt. Please do the needful.
