Policy Bazaar — reg.: aditya birla health insurance policy no.[protected]

I have purchased policy no.[protected] on 17/2/18 from policy bazaar. I have declared that I am diabetic patient and for that, I have submitted all the proofs like my id proof and my family members' id proof as well as my medical report which suggests that I am a diabetic patient.
I don't know policy bazaar agent has committed some mistake and it hasn't been reflected in the policy, for this constantly I am trying to contact and tell them to resolve but they are not resolving. Even I have submitted all these documents thrice to them as per requirement, but still, it has not been resolved. They(Policy bazaar) are saying that from Aditya Birla side below mentioned documents are required;
1. new Proposal Form
2. GST Annexure
3. Id proof of proposer and all insured members.
4. cancel cheque of proposer's account

Kindly understand it's not my fault its a matter between you and policy bazaar, so resolve it. Because at the time of purchase only I have declared it. It was the fault of some agent. Now I am not free to download and fill all the detail in the form, then after the scan it and mail it. also, if I would have to do all this then I would not have preferred such intermediary to purchase the policy, rather I would have directly purchased.
From both side I have been harassed, I have already paid the money. At the time of payment, no one is bothered about the signature of the client even then why this thing has been stretched unnecessarily.
If the matter is not been solved, then definitely I will go to consumer court against both policy bazaar and Aditya Birla health insurance both as no one trying to resolve the issue.

Even you can activate online portal for me as at the time of purchase you have provided to fill the data. I can do that but to resolve the issue whatever been remedy been suggested to me that is just unacceptable as its the fault of the agent of Policy bazaar.
