Public Sector Insurance Co. — refund of waiting list ticket

Hi sir,
I took tatkal ticket for travel on 11may, in Train number 13352, class:3A: and boarding point chennai central to ranchi. But waiting remain unchanged for the tickets that i had.
Once the chart prepared..waiting list for PNR number:[protected] was waiting list 1/2/3 for me(Ranjita Kumari), son and my mom. More details are available on the ticket attached. Since waiting was not confirmed.i could not travel. It was around 11 pm so i have returned back to home immediately for our safety. Next day (12may2019)morning at 8am i reached railway counter and once filled up cancellation form wasn't given the refund.
Since it was late night and was with my elderly mom and Son so for our safety..we have returned back home and tried cancelation of our ticket from next day morning onwards..

I am request for refund.
If required will share ID proof for all of o[censored]s.
