Rajiv Gandhi Proudyogiki Vishwavidyalaya [RGPV] — Exam form not forwarded

Hi Sir,

My name is Ayansh Deshmukh, enrollment no. 0827AL211020. I am persuing Btech from Acropolis Institute of technology and research, Indore. I want to fill the 2nd semester exam form but it shows not forwarded by the college.
I have talked with the HOD and explained my situation, i was diagnosed with Johndis and was severely ill due to which my attendance is short, I have all the medical certificates and I an ready to submit them, as 7th of July is the last date, I request you to do something for getting my form forward.
