RTO India — learning licence not received [protected]

Sir,
My daughter applied for learning licence through your online website on the date 03.09.19 with application number [protected] and furnished all the formalities and got slot timing. She reached your office to complete all the other formalities as described in slot timing and completed photo etc in time. After so many time she still hasn't received any information from the office. After 3 visits to your office they said the concerned clerk forgot to fill your test report and asked me to get another slot timing and come again. But, now there is no available slot date till a very long time.
Please let me know that what was my mistake and why i have to suffer. Please rectify my problem and make my learning licence.
