Form 5 is a Driving Certificate issued by Driving School of Establishments. It is typically required when applying for a driving license, particularly for commercial vehicles.

I have contacted RTO office, KA-03, Bangalore East, registered complaints online, written email to RTO and contacted [protected]@gov.in and no one has given any solution.
