Student — I forgot my employment user id and password how to I get

I am cinthamani i have completed MBA ON 2013. then i renewal the employment for online. my user id is CHP2015F00005099 DOB ID 04/05/1991. This year renewal date so i enter the id and pass word but it is not enterd one box will opend your user id /password incorrect . pls help me how to i renewal
