Student — not forward my form by institution

My name is "SURAJ PRAKASH" and I am second year student of b.tech from ECE branch and my scholarship registration no.([protected]) is this
my scholarship form is not forward by institution
BECAUSE
my some details fill any another student like ( account number, aadhar number, enrollment number, roll number, mobile number, branch, and like that )
falting of by my institute, which that, wrong form forward without seeing O[censored]TKARSH VERMA and registration no.([protected]) so,
my scholarship form is not forward and " institute and पिछड़ा वर्ग कल्याण विभाग उ0प्र0 not take any action "
please sir my problem kindly resolved
