Syndicate Bank — Currency note damaged 500 rupees note from syndicate atm

Dear sir:
I had withdrawl rs 10000/- through syndicate atm (id a8781002) on 04/11/16 at 09:05 through my obc visa debit card no [protected] vide txn no 2463 against my obc saving bank account.
Currency delivered were:
Used notes of: rs 500x19 =9500
Rs 100x5= 500
Total rs 10 000/- only

One of the 500 rupee note bearing no 8 aw 244 670 (scan photo enclosed) lying with me is damaged with large cut at one side. this 500/- rupee note was refused in market. at the time of money withdrawl, no person was available around. i searched for some foe quite sometime, but failed.
I shall be advised, to whom should i contact, to exhange my 500 currency note.
