Uco Bank — Non acceptance of 15G

Dear sir,
I went to durgapur city center uco bank branch to submit 15g. I have visited four times to submit form 15g. I am long term relationship with uco bank and i am also holding some fixed deposit with them. Without any reason they have denied to accept form 15g. Four times i have visited and the time was 3pm, 3pm, 3pm and 1:30pm. If this is the service of the bank like uco then i have to close my entire relationship.
