This is to inform you that a transaction of Rs. 1000.00 has been made by me on 12.02.2019 through ATM. Transaction is not completed but the said amount is being debited from my account. Copy of screen shot is enclosed for your ready reference. The reverse amount has not been credited as yet.

I, therefore, would like to request you to kindly reverse the amount as stated above at the earliest.
