Uti Mastershare Plus Unit Scheme 1991 (Master Plus) — Non receipt of any communication about dividend declared, new folio nos, if any

I have been allotted 500 units of Master share plus unit scheme 1991 under regd folio no. 0850576 dt[protected]. Scheme changed to many new names. viz-- UTI leadership Equity Fund, Bluechip Flexicap Fund etc. Total units last was converted to 2085.71. Last dividend received was in 06 March 2017 . Thereafter no communication . No statement from the then registrar KARVY COMPUTERSHARE PRIVATE Limited. No intimation about pending dividends or new share certificate etc. Its old small investment . So made without PAN, email Id . There is one anomaly in name of Investor too viz. Investment was made with First and Middle name (i.e) without surname, while bank a/c and PAN shows surname too. Though My Aadhar is also without surname. Even then my PAN no. has been linked with the Aadhaar and is KYC compliant. Yet my investment was made without surname. So may trouble in KYC
