Sir, i have proposed for lic policy amulya jeewan in the title name of my wife manpreet kaur for rs. 25 lacs on sep’2015, for which i have paid premium of rs. 14136.00 vide chq no. 24 dt.[protected] of dcb bank, ferozepur city. after that lic advised me for some medical tests which were necessity for this policy. sir i went to amritsar with my wife & done all the necessary formalities.

Waiting for one month i asked lic agent...

I am lic agent i lost id pasward please resent ajent pirtal id pasward My agency code 03574198 Nam rameshwar lal balai Branch bhilwara...

dear sir ,i have my policy lic market plus no.[protected] from LIC rudrapur branch of Rs.50,000/=

when i go to lic office for surrender this policy, i found that the above policy was surrender on 19/03/2010 by L.I.C.

and they are not given me any proof for that surrender.

We submit the death claim Form at L.I.C. Ajmer office and get the claim for Rs. 812.00 as under:

i. THAT I AM, A SENIOR CITIZEN, SUFFERING FROM A NUMBER OF PHYSICAL AILMENTS. I CAN NOT AFFORD THE MEDICAL EXPENSES THAT MAY RUN INTO LACS OF RUPEES.
.ii. THAT THE LIFE THREATNING DESEASES CALL FOR IMMEDIATE TREATMENT AS ADVISED BY THE DOCTORS WHO ARE PRESENTLY TREATING ME. THE COPIES

i have already sent one complaint regarding tr.of my police.no[protected] &81809003 to ujjain br. from shajapur br. aglongwith full details of loan of policy.no.[protected].and i ask you why you did not sent any reminder within 9 to years about loan of my policy no.[protected].and till date nobudy take any action and not gie any reply or responce.pl. take necessary action and advice me.thanks...

I PURCHASED THE ABOVE POLICY FROM LIC -DHANBAD OFFICE ON 06/02/2008.ON 23/07/08 AT INDORE I WAS INJURED IN A ACCIDENT AND ADMITTED TO THE SNG ORTHO HOSPITAL, INDORE FOR OPERATION OF MY LEFT ELBOW. UPTO TILL NOW SEVERAL REQUESTS HAS BEEN MADE BUT NO ANY ACTION TAKEN BY LIC.MY HEALTH CARD ALSO NOT PROVIDED TO ME TILL DATE.

THEREFORE YOU ARE REQUESTED KINDLY ARRANGE FOR MY PAYMENT OF MEDICAL EXPENDITURE AS PER HEALTH PLUS PLAN...
