I recvd the maestro card on 21/05/2009 State Bank located at Madame Cama Road Mumbai (8586). My Saving Bank A/c No is [protected]. The bank had taken my signature on the document of well come kit. The Pin code for the above Maestro Card No[protected] was found missing. They are showing me the register that you have revd it. Told me to give application to receive the same.
