I HAVE INSTALED WITH TWO DIGITAL DISH ON SAME ADDRESS BUT I HAVE BEEN GIVEN ONLY ONE CUSTERMER IDWHICH IS [protected] ON NAME RC SRIVASTAVA OTHER FORM WAS ON CP SRIVASTAVA SO IT REQUESTED TO ISSUE ANOTHER FOR OTHER PERSON .
