Dear Sir,
I Haresh Chainmal Nagdev hereby requested you to please cancel my policy No.[protected] . On 9th January I went into the branch office and meet with agency development manager Mr. sanjay vaswani for cancellation of the policy but he force for non cancellation of the policy and also he kept my original policy document with him for taking boss Pankaj Sinha approval for cancellation of the policy. On 12th January I went into the office and the boss was not in the office full day .On 13th January I meet with boss pankaj sinha then he said first you meet with training manager Mr. Ganesh Iyer after that you will cancel the policy .On 14th January I meet with ganesh iyer and after meeting I was said to the boss to please cancel my policy but he said you come on 16th January with ADM sanjay vaswani. On 16th January I went into the office with sanjay vaswani then boss pankaj sinha received my original policy document from sanjay vaswani and written letter from me without giving any receipt and also said you will come on 20th January then your policy will become cancel surely and the receipt will be given on that time. On 20th January the boss was in full day training meeting . At 7:00 Pm I was call on phone to the boss then he said you will come on 23rd January at 12:00 pm .On 23rd January at 12:00 pm the boss suddenly start PRP meeting when he seen me. I wait about 1 hour and 15 minutes after that I went into the dombivali branch. In dombivali branch also the cancellation were not accepted . After that I return into the ulhasnagar(amu 34) branch at 4:00 pm then also the boss pankaj sinha not accepted the cancellation of the policy due to business leakage.
I already gives complaints to the customer care and agent care service centre on 11/01/12 reference no. 7959944 , on 12/01/12 reference no.7970779 , on 16/01/12 reference no.7995376 and also sended Fax and emails on 23/01/12.
So I am hereby further requested you to please cancel my policy and refund my premium as soon as possible.

Max New York Life Insurance Co. Ltd. — cancelling the policy #777992082

I(ravinder dutt tyagi) purched the max new york life insurance policy no. #[protected] but this policy have misselling to me.So i requested the canceled the policy on 16/05/2010 but i have not received the fund till now.
I have requested to you (max new york life insurance co. ltd.) please refund the fund as soon as possible.

Max New York Life Insurance Co. Ltd. — Cancellation of Policy

Respected Sir/Madam,

This is in reference to the policy number[protected] received on 12/10/2009. The policy was incorrect as there are some discrepancies as mentioned below.

1) I had opted for one time premium but we received a 10 year premium policy, which is unacceptable.
2) Application is NOT signed by me.

Therefore, I had emailed them on 12/10/2009 itself [reference number for the email sent on 12/10/2009 was: 1173397] to inform them about the above mentioned discrepancies in the policy and thus I had sent the policy back to them on 13/10/2009. I had even asked them to rectify the policy and send me the revised policy.

But there was no response from them for about 3-4 months and therefore after getting frustrated I emailed again on 17/3/2010 [reference number for the email sent on 17/3/2010 was: 2146167] as to cancel my policy and refund my Rs. 100000.

In response to that, I received a letter on May 29, 2010 stating that every member is given a 15-day free look period to see the policy and cancel if needed and as that 15-day period is over there is nothing they can do about it now.

So I emailed them on July 7, 2010 stating that "I would like to inform you that the policy documents has been manipulated by the agent or someone in your company as the policy documents does not even hold my signature. The signature in the documents are BOGUS and the term mentioned in the policy is 10 year but we had opted for ONE TIME PREMIUM policy. It seems that someone has filled a new form with 10 year premium and has forged my signature. According to this wrong fake policy, I will never be able to take my money back even after 10 years as my signature will not match with the form. So please take some severe action on the agent or person responsible for forging my signature and changing the forms." but there was the same response from them as described earlier.

Please help me in this case. I will be highly grateful and obliged.
