Pran kit not received

Respected sir,

I am an employee of Union Bank of India, working in Kanpur. Due to change in my residential address which was after submission of the form, I could not receive my Pran Kit on the address mentioned by me in the form. I have been allotted Pran number which is: 110012404205.

I therefore request you to kindly re-send my Pran Kit to the below mentioned address which is my official address:
117/H-1/240, Pandu nagar, Kanpur-208005.

I had applied for my pran &received a message of its generation on 23/12/10 but has not got the kit till 19/2/13.while querieng got the prank no as 110021193228 & was sent to special treasury berhampur ganjam odisha. I stead of se ding the pran kit to my sword's ie sdvo berhampur the treasury has sent the kit to senior suptd, circle jail, berhampur ddo regd no sgv096331e .hence my case may kindly be solved sympathetically.as I am suffering with higher income tax deposit for the last 3 years
