[Resolved]  Provident Fund Redemption — PROVIDENT FUND REDEMPTION

Dear Sir/Madam,

Pleased to inform you that I submitted Form 19 and 10 (c) of my EPF No. MH/34020/003643 for radeeming my EPF through registered post on 13/11/2009 and also the cancelled cheque of ICICI Bank, MG Road, Ernakulam (A/C. No.[protected] in order to credit the amount directly into the Bank to REGIONAL PROVIDENT FUND COMMISSIONER, BANDRA (E), MUMBAI. My last employer was M/s.Bajaj Allianz Life Insurance Company Limited. I relieved from there on 30/06/2009 and I am not availing EPF currently.

I am very much in need of money due to my father's hospitalisation (acute liver problems). Kindly consider this as a special case and radeem my above mentioned EPF and directly credited to my bank account as early as possible.
