I have purchased preventive healthcare membership plan (gold), paid on 16 may 2018, receipt no: AHC[protected] and received kit on 22th may 2018. On that day onwards i have requested service for master health check-up for MY Father and mother. Since i didn't receive the proper response from concern and i decided that don't want to waste my money which paid access 2 health care for preventive healthcare membership plan (gold).i paid

So due to unsatisfactory service, as per their terms & condition 12th point, i have initiated my refund amount as soon as possible and
