Sir, I no.670129r sgt gopal krishna deposited rs-600/- vide receipt no.1779 by cash on 01/06/2017 at afa up kanpur towards lifemembership identity card for onward submition afa head office new delhi. Afa up forwarded my details to afa head office new delhi through email on 20 nov 2017 for making identity card. Till now i did not receive it. Kindly pursue this matter.
