Here with Informing that I have submitted two claim simultaneously the details is as under. (1) Mediclaim for bill of Sun Rise Hospital-Kolhapur period for[protected] to[protected] Amt. Rs. 20291=62 (2) Mediclaim for bill of Sun Rise Hospital-Kolhapur period for[protected] to[protected] Amt. Rs.18630=52 Today I have received only one claim no.90588055. I have not received any information regarding my second claim.I have submitted both claim in one file but in separate form. (Note : Pl. refer my letter of 1st March 2015 at the time of submission of claim) Please refer my letter[protected] in that letter I had submitted copy of both claim. Here with again I am submitting copy of 1st letter/2nd letter of[protected]/copy of my both claim/medical bill details of my both claim.
