Patient Name: Prafulla Ch. Tripathy

Address: Baijamunda, Sambalpur

With reference to the subject, I would like to highlight your gross negligence towards patients by which someone's life is in stake. First you go through the attachment, all the references regarding my complain is there.
My father Mr. Prafulla ch. Tripathy was admitted on 07.06.16 to your hospital after complaining about chest pain. You had conducted angiography( Dr Lingraj Nath) and found one blockage in heart and put one stent on 08.06.16. And he was kept in ICU for 3 days. On 10.06.16 your radiology Dept Personnel Dr. Ramakrishna Meher had conducted ultrasound of my father, plz find the report attached and go through it properly. You have written in technical language that he had some issues( LARGE HAEMANGIOMA ON RIGHT LOBE OF LIVER) in his liver. We being naive to such terms, we couldn't understand what's written there. However it was your duty to advise us that there were some issues n needs attention. Neither the radiology department, nor the doctors told us anything in this regard. Even in the discharge summary, no advise was there to follow up or anything.
After a month, in July 16, they had visited for follow up, still doctor never advised to check up his liver, again in August he had visited for follow up, still the same casual attitude & negligence from your side and doctor never mentioned anything.
However my father's health started deteriorating with weight loss, loose motion, stomach ache, body pain. We visited several doctors in sambalpur, bt only one doctor doubted with cancer and advised to go to Bhubaneswar for further CT scan.On 06.10.16 we conducted ultrasound and CT scan and it was confirmed that he has HCC liver cancer since long and hence all the symptoms.

Have you been little serious enough in your job and could have advised us regarding his issues with liver in the month of June, he would have been treated earlier when the disease was at early stage and would have been recovered by now. This is to inform you that he is in Bangalore now under treatment and is really in pain and situation is critical. Only the doctors in Bangalore told us that it was clearly there in the ultrasound dated 10.06.2016 and we have not treated it earlier. It was really a surprise for us to have heard this from them that the doctors in AMRI hospital are so casual and care a damn about the patient’s life. Now could you let us know that the who would bear the pain, the trauma, the expenses and the uncertainities we are going through right now. How could a institution like yours be such casual and negligent ??

I want a proper explanation from your side on gross negligence and this mail is only to warn you in advance before I take legal proceedings on your hospital.

Plz take this mail seriously else be ready for the consequences.

I am going to put these in Social Media, so that everyone should be aware and will continue to do so till I get any explanation from your side.

Regards,

Nibedita Tripathy
